RANK ONE IS NOW OPEN – ALL RANK ONE FORMS MUST BE DONE BEFORE AUGUST 14TH!

Suffolk County Green Key Form:

All players are required to have a valid Suffolk County Green Key card to play Suffolk County Golf Courses: Bergen Point, Timber Point, West Sayville and Indian Island. We will be playing 3 county courses during our season.

If you do not have a valid Suffolk County Green Key card you can get information on obtaining one here: http://www.suffolkcountyny.gov/Departments/Parks/GreenKeyCard.aspx

When you have your valid Green Key Card, please download and fill out the Green Key form HERE. You can fill out the form on your computer and then print and sign it. If you do not have access to a printer, fill out the form electronically and email it to me at bkroll@babylonufsd.com and I will print it for you and you can sign it when I see you.

School Physical:

If you choose to have your physical with your own doctor, you must have him/her fill out the school’s form.

Physical forms can be submitted in one of the following ways:

  • Bring it to the Attendance Office over the summer
  • Scan and email it to gmchugh@babylonufsd.org
  • Fax the form to (631)893-8021
  • Mail the form to: Babylon Junior Senior High School, Attn: Health Office, 50 Railroad Ave, Babylon, NY 11702

PHYSICAL FORMS MUST BE RECEIVED BY AUGUST 14TH!

All athletes must be fully immunized in order to be eligible for sports participation
